High Holy Day Ticket Order Form and Attestation of Vaccination Status

We look forward to welcoming everyone back to BZBI in the safest and most accessible way possible. When planning for the quickly approaching High Holy Day season we are prioritizing the health and safety of all our community members. Barring any drastic changes in COVID-19 infection rates over the next month, we will return to BZBI for in-person services on Rosh Hashanah and Yom Kippur. All services will also be accessible online via Zoom and Facebook Live. Parents with children can reserve spots for babysitting or Youth and Family activities by clicking here.
 

IMPORTANT - WE REQUIRE BZBI MEMBERS WHO PLAN TO ATTEND IN-PERSON SERVICES TO COMPLETE THIS FORM BEFORE 12:00 NOON ON FRIDAY, AUGUST 20, 2021.


Adults are defined as people living in your house who are age 23 or older, and not in school.

Because services will be multi-access this year, you have the option to attend one, many, or all services online.  Note that adult services are available both in-person and on Zoom, but Youth & Family events will only take place in-person.

Access to all adult services on Zoom during the High Holy Days are offered at no charge to BZBI members, those who fall under their membership, and their guests. To register for access, click here for Zoom registration.

You may reserve one or more copies of our High Holiday Machzorim for use in your home, available at no charge with a refundable deposit. Orders for Machzor loan are due by noon on FRIDAY, AUGUST 27, 2021. With your confirmation email, we will send information on pickup dates and locations.


Families who want childcare and/or have children who want to attend Youth & Family services must register in advance for these programs using a separate and required form located here.
Current guidelines suggest that we will not have room for guests at in-person services. We encourage extended family and out-of-town guests to join us on Zoom. However, we will notify you if guest tickets become available for the names you add here.

All members of your party age 12 or older, who are eligible, must be fully vaccinated to attend Sanctuary services.


The potential symptoms of COVID include:
    Continuous or intermittent cough
    Shortness of breath
    Fever or feel feverish (with or without chills)
    Sore throat
    Headache, muscle aches or muscle pain
    Recent loss of the sense of taste or smell
    Fatigue (tired)
    Congestion or a runny nose
    Recent nausea, vomiting or diarrhea
    Recent infection requiring prescription medication
    Exposure in the last 14 days to someone with a cold, flu contagious illness, or suspicion of COVID-19.


Our goal is to maximize the number of members who can safely attend High Holy Day services in person, while still following all CDC and government guidelines. This intention requires us to implement the following policies:
All who attend in person services over the age of 12 must be fully vaccinated. An exception will be made for those under 12 who will be allowed to attend without vaccination.
Those over the age of 2 must be properly masked at all times while in the building, with the exception of specific individuals on the Bimah.
Anyone who is medically unable to be vaccinated or immunocompromised should participate in services via Zoom, not in person. We are committed to providing a quality Zoom experience for anyone who cannot attend in person.
We urge anyone age 12 or older who has not yet done so to get vaccinated as soon as possible.
Should the situation with COVID-19 change dramatically, we will of course need to adjust our plans and we appreciate your patience and understanding.
